Home Delivered Meals Program

"Meals on Wheels"

Eligibility and General Information

Clients are assigned a caseworker:

Sharon Cowden and Sonya P. Moody	Aiken, Graniteville, Jackson, New Ellenton, Valley area and Wagener

Caseworker will visit client to make an assessment for eligibility or client is referred by Community Long Term Care (CLTC).
Eligibility

assistance from family members

being homebound

unable to prepare meals

	Charge of meal is based on income and available funding. The cost per meal is $4.00 for "paying" clients.
	If client lives in rural area, they may only qualify for Shelf Stable Meals.

Shelf Stable Meals

A supplement for a Home Delivered Meal for seniors that live in a rural area:

a nutritious meal where a a hot meal is not an option

    Information Request Form

     Have Caseworker contact me in reference to applying for home delivered or Shelf Stable meals.
     Have a Volunteer Program Coordinator contact me about delivering meals.
