CLTC offers programs to help individuals who want to live at home, need assistance with their care, and are financially eligible for Medicaid.
The Lower Savannah Council of Governments (LSCOG) has a grant under the Family Care Giver Act to provide relief and support to the primary caregiver of a frail or disabled adult and provides a break for the caregiver from the continuous responsibilities of caregiving. Without this support, the primary caregiver's ability to continue in his or her role would be jeopardized; thereby increasing the risk of institutionalization of the frail or disabled adult.
